| Instructions for Financial Affidavit This form is the Pima County version of Form 2, ARFLP. It is also downloadable in Word and Word Perfect. This form is referenced in ARFLP Rules 47, 49 and 91. This form is used in family law cases where financial information is required. A typical case is for child support or spousal maintenance.
Prepare the form online and then print it for best results. If you choose to print a blank form and write answers, type or use an ink pen. If a space does not apply, write "N/A". If an income or expense line does not apply, mark a "0". Note: There are additional instructions in the body of the form.
If you need more space for an answer, attach a supplement and refer to it.
Attaching your pay stub is not a substitute for filling out the form. You are welcome to attach it as helpful information. However, be advised, the documents you file with the Clerk are public records and available worldwide. Black out identity numbers on your attachments. To provide sensitive financial data, click here and use Form 3, ARFLP.
Do not worry about the on screen instructions. When you print, the on-screen instructions will not print.
You may be required to bring the completed form to a court hearing or file it in advance with the Clerk of the Court. To file with the Clerk, mail the completed form to:

Be sure when you bring the original to court you have at least two full copies with you. If you file a copy with the Clerk, you must also mail a copy to the other party's attorney. If the other party does not have an attorney, mail the copy to the other party. |
