Instructions for an Annual Report of Guardian
Answer all questions. If a question does not apply, write "N/A" in the space for the answer.
If you need more space for an answer, attach a supplement and refer to it in the space for the answer.
You can attach medical, financial or social service reports and refer to them in the answer.
You must read, give or mail a copy of the report to the ward. You must tell the court on Page 4 of the Report that you did so. What, if anything, the ward does with the copy is up to the ward.
You must also mail or deliver a copy of the report to the following persons: the ward’s conservator (if any); the ward’s spouse or the ward’s parents if the ward is not married; any court-appointed attorney for the ward (unless the court has discharged the attorney); and any other interested person who has filed a demand for notice with the court.
All co-guardians must file a report. If you are co-guardians, you may both sign the same report and submit it as a joint report. If you do not sign the same report, you must file a separate report and mail a copy to the co-guardian. In such cases, you must tell the court on Page 4 of the Report that you did so.
An Annual Report of Guardian is a confidential document, and must be accompanied by a 9”x12” manila envelope with a Confidential Filing Label affixed to the envelope.
Mail the original, signed Annual Report and the manila envelope to the Clerk of the Court. Include a copy for the judge or the commissioner to review. Mail the documents to:
