Instructions for Notice of Death of Ward

  1. If you choose to print blank forms and fill in the blanks, be sure to type or use an ink pen. In addition, make all choices and cross out all areas that do not apply.

  2. The forms are designed for the simple case: The ward had little or no assets. You do not serve as a conservator for the ward. There is no one to whom you are required to give legal notice. You do not seek or require release from potential liability for prior acts or accounting of funds in your control. In the simple case, there is no court hearing. The Clerk of the Court sends your completed forms to a judge for review and approval. You will receive the answer to your request in the mail.

  3. Don’t forget to attach a copy of the death certificate. Be sure you sign the Noticed and Request.

  4. Don't forget to complete the FORM OF ORDER to submit with the Notice.

  5. Mail the completed forms with the attachments to:

  6. The Clerk charges a fee for certain filings. The Clerk's web site, http://www.cosc.co.pima.az.us/, provides fee information, phone and email contacts for your convenience. Fees are subject to change. Check with the Clerk's office to verify actual fee amounts.
