Instructions for Consent and Waiver

1. The form is used with an application for appointment of a guardian or successor guardian of one or more minor children.
   
   
2. If you choose to print blank forms and fill in the blanks, be sure to type or use an ink pen.
   
   
3. Answer all questions. If a question does not apply, write "N/A" in the space for the answer.
   
   
4. A minor who is fourteen (14) years of age or older must also complete a consent form.
   
   
5. A parent or minor (14) years of age or older may also appear in open court at the time of the hearing and state in open court their consent.
   
   
6. Bring the completed form to the hearing or mail the completed form at least twenty-one (21) days prior to the hearing date to:
