Instructions for Proof of Restriced Account

  1. The form is used for conservatorships of adults or minors when a restricted account is ordered by the court. The court order has a date by which you must submit the form.

  2. An authorized representative of the financial institution must sign the form. Make sure the representative tells you they have authority to sign.

  3. A separate form must be completed for each financial institution if more than one institution is used. A separate form must be competed for each minor if there is more than one minor in the conservatorship.

  4. If you choose to print a blank form and write answers, be sure to type or use an ink pen.

  5. Fill in all spaces. If a space does not apply, write “N/A” in the space for the answer.

  6. The form is designed for the simple case: The funds are held in one or more restricted accounts at a local bank, savings and loan, or credit union.

  7. Do not forget to take the filled in form to the bank officer to complete and sign.

  8. Mail the completed forms with the attachments to:

  9. The Clerk charges a fee for certain filings. The Clerk's web site, http://www.cosc.co.pima.az.us/, provides fee information, phone and email contacts for your convenience. Fees are subject to change. Check with the Clerk's office to verify actual fee amounts.
