Instructions for Proof of Restriced Account
The form is used for conservatorships of adults or minors when a restricted account is ordered by the court. The court order has a date by which you must submit the form.
An authorized representative of the financial institution must sign the form. Make sure the representative tells you they have authority to sign.
A separate form must be completed for each financial institution if more than one institution is used. A separate form must be competed for each minor if there is more than one minor in the conservatorship.
If you choose to print a blank form and write answers, be sure to type or use an ink pen.
Fill in all spaces. If a space does not apply, write “N/A” in the space for the answer.
The form is designed for the simple case: The funds are held in one or more restricted accounts at a local bank, savings and loan, or credit union.
Do not forget to take the filled in form to the bank officer to complete and sign.
