Instructions for Demand for Notice

  1. If you choose to print a blank form and write answers, be sure to type or use an ink pen.

  2. Fill in all spaces. If a space does not apply, write “N/A” in the space for the answer.

  3. The form is designed for the simple case. Any interested party can request to be kept informed of all pleadings filed in a case by filing a demand for notice. Once the demand is properly filed and notice mailed to other parties, the parties in the litigation will copy you with their pleadings. When you file this pleading also send a copy to any other party or entity entitled to notice in the case. When sending others a copy of this pleading, state you did so on this form on page 2 of 2.

  4. Mail the completed forms with the attachments to:

  5. The Clerk charges a fee for certain filings. The Clerk's web site, http://www.cosc.co.pima.az.us/, provides fee information, phone and email contacts for your convenience. Fees are subject to change. Check with the Clerk's office to verify actual fee amounts.
