Instructions for Request for ADA accommodations

  1. This form is for use to notify the court and parties of a request for reasonable accommodations for a disability. If you need an accommodation under the Americans with Disabilities Act, use this form to call the need to the attention of the court.

  2. If you choose to print blank forms and fill in the blanks, be sure to type or use an ink pen. Fill in all blanks. If a blank does not apply, enter “ n/a.”

  3. Be as clear as possible when you state your disability and when you identify the accommodation needed.

  4. Be sure to send a copy to the assigned judicial division, if any, and to the ADA Coordinator for the Court.

  5. Mail the completed forms with the attachments to:

  6. The Clerk charges a fee for certain filings. The Clerk's web site, www.cosc.co.pima.az.us, provides fee information, phone and email contacts for your convenience. Fees are subject to change. Check with the Clerk's office to verify actual fee amounts.
