ANNUAL REPORT OF GUARDIAN OF A MINOR

USE THESE INSTRUCTIONS if you have been appointed as the Guardian for a minor and need to file an ANNUAL REPORT OF GUARDIAN. You must complete this form every year you are the Guardian for the Minor. The report must be filled out in English.

TOP OF PAGE: Fill in the information requested about you, the Guardian, at the top left of the form, include the case caption information (name of the minor, date of birth and the case number). Read the "Warning" section and sign your name and date. Next, match the numbered instruction to the numbered line on the form.

  1. Check the applicable box and describe the type of home, the name of the person in charge, the address and who is the primary caregiver.

  2. Answer the questions regarding how many times you have seen the minor over the past year and when you last saw him or her. If the minor lives with you, you may simply state "Lives with me" and "Every day" or "Nearly every day" or whatever is true for your situation.

  3. Provide the information describing any major changes in minor's development in the past year.

  4. List the name of the school the minor is attending and how the minor is doing in school.

  5. (Also # 6, #7 & #8) Provide the information requested about the minor's physical and mental health. Make sure you attach a current copy of the doctor's report (within the past 9 months). You must include the doctor's name, address, phone number, the date of last doctor visit. A written statement from the Doctor that describes the doctor's observations about the minor's physical and mental health is sufficient.

  6. If the minor is a disabled or incapacitated person, state whether or not the guardianship should continue after the minor becomes an adult.

  7. List any services being provided to the minor by a government agency, including social security benefits. Provide the name of the person responsible for the minor's affairs with that agency.

  8. List all persons, including minors, who reside with the minor. If a minor residing in the household is also subject of a guardianship, list the case number of the guardianship.

  9. Report whether any person residing in household has been convicted of a felony. If the answer is yes, explain the circumstances and list the state and county where the offense occurred and the court case number.

  10. Explain whether any proceeding for adoption, custody or dependency for minor has been filed in the last year. If the answer is yes, list the court case number.

  11. Explain whether the minor is enrolled in a health insurance plan or an equivalent program offered by the state, (such as AHCCCS).

  12. Describe any efforts you have made to obtain child support from the child's parents, either in person or through a state agency.

  13. MAILING AFFIDAVIT: Write the names and addresses of the people to whom you mailed a copy of the ANNUAL REPORT and the date you mailed it. You must send a copy of the report no later than 19 days before the non-appearance review hearing to the following people:
    1. The Minor, if 14 or older
    2. The Minor's Conservator (if applicable)
    3. The Minor's parents
    4. The Court-appointed attorney for the Minor (if applicable)
    5. Any other interested person who has filed a demand for notice with the Court.

Signature: Make sure you date the document, and sign your name. If there are 2 guardians - They must each sign the form OR each complete their own form.
