Instructions for Request

  1. The form is designed to request different types of relief after an order has been issued. The form can be used by either party. With this form, you can ask for a hearing, a continuance or a dismissal.

  2. You can print the form or prepare it on your computer. The form works best when prepared on your computer. If you choose to print a blank form and write answers, be sure to type or use an ink pen.

  3. Fill in all spaces. If a space does not apply, write “N/A” in the space for the answer.

  4. Check only the boxes for the document(s) you received.

  5. Do not be concerned about the on screen instructions. When you print the form from your computer, the on-screen instructions will not print.

  6. You may be required to bring the completed form to a specific court hearing or to file it in advance with the Clerk of the Court. If in doubt, file the form in advance and bring extra copies to Court. To file the form with the Clerk mail the completed form to:

Be sure when you bring the original to court you have at least two full copies with you. If you file a copy with the Clerk, you must also mail a copy to the other party’s attorney. If the other party does not have an attorney, mail the copy to the other party.
