Instructions for Petition for Protection Order or Injunction

  1. This form begins the process in Pima County Superior Court. You use the form to ask the Court for protection. If you qualify for a protection order choose that type of petition. If you qualify for an injunction, choose that type of petition. In general, protection orders require a domestic relationship. Domestic relationships may include marriage, former marriage, relatives, common children, parent, grandparent, child, grandchild, sibling, in-laws, step-relations, adoptive relations and cohabitants. See ARS 13-3602. If no domestic relationship exists, use the Petition for Injunction against Harassment. Workplace Harassment Injunctions are a specialized category. See ARS 12-1810.
  2. Do not forget to complete the Guidesheets for the Plaintiff and the Defendant and bring both with you to the Court.
  3. You can print the form or prepare it on your computer. The form works best when prepared on your computer. If you choose to print a blank form and write answers, be sure to type or use an ink pen.
  4. Fill in all spaces. If a space does not apply, write “N/A” in the space for the answer.
  5. If you need more space for an answer, make and attach a supplement and refer to it in the space for the answer.
  6. The form is intelligent. Do not worry about the on screen instructions. When you print the form from your computer, the on-screen instructions will not print. The form will skip unnecessary blocks based on your answers.
  7. bullet_error.pngWarning: The Defendant will get a copy of your petition. If you do not want the Defendant to know an address, do not put it on the Petition. In such cases, use the blocks provided on the Guide Sheet, page 1. However, if the Defendant knows an address or you do not care if the address is known, then put the address on the form. When an address is on the order, the order is clear and the law enforcement response is faster.
  8. To file the form and request an order bring the completed form to the:
