Sign and date page 6; sign or initial, as appropriate, the certification, the “Authorization for Background Checks” and the “Authorization to Release Personal Information” on pages 7 and 8. The release on page 8 requires a witness signature. Please make sure to read the bottom of page 7 about electronic signatures. INSTRUCTIONS FOR LETTERS OF REFERENCE AND TELEPHONE CALLS
In response to the requests for references within section “F. REFERENCES", please provide for EACH question (41-44) the names, telephone numbers and e-mail addresses of three primary references who have a comprehensive knowledge of your qualifications.
If applicable, a reference may be listed repeatedly. The committee will contact these references and such other persons as may be deemed necessary. The committee will not consider solicitation or lobbying by other parties whether by phone, letter, or e-mail. The three references should provide assessments of the applicant's skills, expertise, ethics and any other characteristic relevant to an individual's potential for Commissioner.
Applicants should not personally contact selection committee members regarding their application during the selection process. Committee members cannot individually interview applicants or commit in advance to vote for any applicant.
