NAME CHANGES (Office of Clerk of the Court)
• Petition for Name Change and order for hearing officer’s signature.
• Persons over 18 years of age may file a petition without notice to anyone (form – Application for Name Change – Adult)
• Petitions for children whose parents are divorced require the consent of the non-custodial parent (form – Petition for Change of Name)
• If the Name Change is contested, a hearing is set before the assigned judicial officer.
